Appointments
To make an appointment with a North Shore ENT doctor or audiologist in either office, call 978.745.6601.
Patients are seen by appointment only. Patients with emergent problems will be seen on a priority basis. If you have an emergency situation, please let us know when you call the office.
Be sure to bring your insurance card, medical history form, referral (if required) and your co-pay to every appointment. For more efficient registration, please print out the medical history form, fill it out in advance and bring it with you to your appointment. If you don't have your co-pay at the time of your appointment, you will be billed an additional $10. If you are unable to keep a scheduled appointment, please notify us as soon as possible, but not less than 24 hours in advance of the appointment.
Office Hours
All offices are open Monday through Friday from 8:30 am to 5:00 pm.
Emergencies
A physician is on call 24 hours a day for emergencies. If you have an emergency, please call our office at 978.745.6601. If your call is after hours, our answering service will page the doctor on call.
Referrals
Let us know if you are referred to our office by another physician so that we may share our findings with your personal physician. If your insurance plan requires a referral from a primary care physician, please be sure to bring the referral with you to your appointment at our office. You may not be seen if your insurer requires a referral and you do not have one.
Insurance and Billing
We participate with most major insurance plans and a number of PPOs and HMOs. Patients should bring their insurance cards with them on every visit. As a service to you, our staff will be happy to file your insurance claims if we have complete and accurate insurance information.
Please understand that it is the patient's responsibility to be fully aware of all the provisions of his or her insurance policy. We suggest you pay special attention to your policy's provisions regarding the need for a referral number, the need for a second surgical opinion and pre-admission review. In addition, some policies have a waiting period for coverage and exclude pre-existing conditions. Your insurance company may require that you make co-payments at the time of your office visit. We bill on a monthly basis for any balance due once insurance has been processed. Please keep in mind that it is the patient's responsibility to see that bills are promptly paid.
